About this policy
This policy describes the Simply Smile Studio website and its enquiry and booking links. For information about clinical records, treatment consent or patient-record retention, please contact the studio.

When you send an enquiry
Our contact form asks for your first name, last name, phone number and message. These details are used to respond to your enquiry. Please provide only what we need to help you, and leave medical history, identification numbers and payment details for your appointment or the appropriate patient process.
When you press Send, your message is processed by FormSubmit, which provides a spam check and emails the submission to our team. You can review FormSubmit’s privacy information before submitting. You can also contact us by phone instead of using the form.
Booking and other services
Online booking opens Curve’s booking service. Information you enter there is processed through that service. Review the privacy notices displayed there before providing information.
Directions and review links open external services such as Google Maps. Those services have their own privacy and cookie practices. Website hosting and service providers may also process technical information, such as an IP address and browser details, to deliver and protect their services.
Website action totals
On simplysmilestudio.ca, we count clicks that open online booking or your phone app, and returns from the enquiry provider. We store daily totals on our existing Azure host, without visitor IDs, cookies, page addresses, search terms or form contents in those totals. These counts do not tell us who visited, whether a phone call connected or whether an appointment was completed. An enquiry-provider return does not confirm delivery to our inbox.
This counting respects Global Privacy Control and Do Not Track signals sent by your browser. It does not read or save your enquiry fields. External booking and enquiry providers have their own practices as described above.
Your choices and requests
You may ask about the personal information the studio holds about you, request access or correction, raise a concern, or ask about withdrawing consent. Legal exceptions and record-keeping requirements may apply.
